In 1986, Diane K. Sherwood, LMSW, C-ASWCM, developed The Caregivers’ Exchange in a non-profit organization serving the Wall Street area. The Caregivers’ Exchange was one of the first eldercare programs of this nature in the country. Three years later in 1989, Ms. Sherwood started a private practice based upon the same principles. Sherwood Associates, Inc., an eldercare resource planning service, expanded and was incorporated shortly thereafter. Client-centered, the practice still focuses upon enhancing the quality of life and extending the duration of independent living of well, frail and disabled elderly.
A skilled, dedicated and stable team of care managers and support staff provide comprehensive geriatric care management services to seniors, their families and guardians. These services include assessment; long and short term planning; effective linkage to medical, legal, homecare and housing services; coaching and counseling; strong advocacy in medical and nursing facilities, with Medicare, Medicaid and health insurance companies; crisis intervention; ongoing family surrogate services and peace of mind backup; payroll support and fiduciary services.
Care management interventions are determined by client need and range from one-hour consultations to full family surrogate services. For highly discerning families who wish to maintain control without the burdens of detail management, we assist with setting up a care team which maximizes the goodness of fit between client and caregiver and coordinates payroll through Paychex. Expertise in dovetailing private funds and public resources and entitlements enables us to also serve those of limited means. Our clients are referred by families, guardians and providers appreciative of our creative problem solving, attention to detail and personalized service. |
